Self-Assessment 1-5 Radio Buttons

I want to create a self-assessment where each question is answered on a 5 point scale ranging from 1 = Strongly Disagree through 5=Strongly Agree. There is no right or wrong answers. Is there some way to do this, perhaps with a horizontal set of 5 radio buttons that is clear, simple and compact? Thanks! --Lbeaumont 02:43, 4 February 2012 (UTC)
