More information needed
editThis is interesting, but leaves some unawsered questions:
- What do you expect to happen with the video?
- Where are existing examples of the videos from this project, if any? if there are none, explain where and when they are likely to be found.
- Other than model release, you presumably want the video and audio released as cc-by-sa to be compatible with Wikimedia projects. This needs stating in the text and explaining to participants.
- You need to explain how the model release / copyright licence document is attached or filed with the video files into WikiCommons or similar.

- Can you clarify which video you're referring to? OpenSpeaks is a toolkit -- OERs to help any archivist make audiovisual documentation of a language. OpenSpeaks does not create video documentations by itself, not at least in a large scale. When it does, they are mostly used as example.
- There are some on Commons (we only use CC-BY or CC-BY-SA licenses for such videos to make them compatible for Wikimedia projects and interoperable with other platforms) You can find some of the example videos on the Commons category "OpenSpeaks".
- Explained about the license above.
- The model release are not exclusive to Wikimedia projects. In fact, we will clarify in the upcoming version that users need to use the Commons OTRS form to keep it simple while taking permission from non-Wikimedia users and uploading on their behalf. But, if a copyright holder is uploading the file by themselves, they don't need to attach the permission form. The model release is a mutual agreement between the person who appears in an interview and the person who is recording the interview. It is not a public document and it cannot be as it might contain sensitive personal information that are key to the particular release. At most, some data might be redacted for privacy for its usage as a proof while submitting to OTRS, when agreed by both/all parties of courses.
I hope I answered your question. Again, the model release is for those who are using OpenSpeaks as a resource hub. They can download, modify and use it however it may seem appropriate for their own workflow. As one of the key creators/maintainers of this project cannot prescribe how each individual user of these resources will make use of it.
