I don't think you should nudge a patient on the sternum for RHombergs. its a test for proprioception and will be positive in someone with proprioceptive sensory loss. If you nudge the patient, its modifying the test to look for cerebellar dysfunction (?more sensitive than tandem walking) in which case the patient should be nudged side to side, not back and forth. I'm not 100% sure about this (just a med student, but a consultant told me this very recently- hopefully someone can confirm/deny because it looks bad in a osce if you do a test wrong, it shows you don't really understand why you're doing it)
