LMCC/Regular Visits

Schedule

• newborn
• 1 week post discharge
• 1, 2, 4, 6, 9, 12, 15, 18, 24 months
• yearly until age 6
• every other year until age 11, then yearly until 18

History

• pregnancy/neonatal history
• feeding/diet
• immunizations
• developmental assessment
• growth, energy, appetite, sleep and review of systems
• past medical history, medications, allergies, family history and social history

Physical Exam

• Growth parameters:
  • serial height
  • weight
  • head circumference (until age 2)
