This section is where you record if adverse events took place, such as hospitalization or arrests, and the changes seen in the youth's mental health and overall functioning (if any). This section is optional, but is appropriate for youths in Wraparound for at least 6 months. Each item must be scored alongside with the providing proof.

  1. Has the youth's living situation been stabilized?
  2. Has the youth visited the ER within the last 6 months?
  3. Has the youth seen a decline in mental health issues within the last 6 months?
  4. Has the youth seen improvement in interpersonal functioning?
  5. Has the youth regularly attended school in the last 6 months?
  6. Has the youth improved in their school functioning?
  7. Has the youth been arrested in the last 6 months?
